About the Role

• Health Professionals and Support Services Award 2020 Level 2 From $79,925-$89,410 pa, or Nurses Award 2010 Level 2 (Above Award).
• Location: Darwin

As a Mental Health Clinical Coordinator, you will have a background in Occupational Therapy, Social Work, Nursing or Psychology. As part of the clinical team, you will be part of a multidisciplinary group who provide clinical mental health assessments and facilitate access and connection to appropriate clinical and community services.

Core duties

• Undertake triage and assessment of mental health referrals
• Assess and screen referrals using appropriate tools, policies, and procedures
• Maintain accurate individual consumer files and databases in accordance with the policies and procedures of Neami and the service agreement with the funding body.
• Provide information about mental health and available services in the community
• Support the continuity and consumer care and information flow between consumers, staff GPs and external workers and other agencies
• Provide individual clinical interventions as appropriate/required
• Participate in additional clinical responsibilities as determined by need and in consultation with the Clinical Service Manager.
• Provide leadership, secondary consultation, case review and support to other staff including intake workers and other clinicians

What will you bring to the role?

As a Mental Health Clinical Support Coordinator you will have a background and be registered in your field of Occupational Therapy, Social Work, Mental Health Nursing and/or Psychology, along with demonstrated experience. Our ideal candidate will have a deep understanding of the issues faced by individuals in the region including those identifying from a CALD or Aboriginal and Torres Strait Islander background.

• Excellent written and strong communication skills
• Active listening and high level of interpersonal skills
• Knowledge and experience with Tele health
• Knowledge of local services particularly relevant to designated program area
• The ability to form and maintain strong relationships with key stakeholders
• Be able to work with individuals from diverse and unique backgrounds
• Be an honest and transparent individual

• Valid Blue Card
• Australian Working Rights
• Drivers licence
• AUS Police check
