About the organisation

North East Community Support Group Inc (NECSG), is a non-profit community-based organisation in Zillmere delivering three main service programs: Zillmere Community Centre, Zillmere Family Accommodation Program and Zillmere Young People Support Service. Our mission is to provide quality community support programs in line with changing needs and services targeted to alleviate poverty, homelessness, distress, disability, isolation, and unemployment.

NECSG has supported families with accommodation since 1985. The Zillmere Family Accommodation Program (ZFAP) is a Specialist Homelessness Service funded by the Qld Government, providing temporary supported accommodation to families who are experiencing homelessness or at imminent risk of homelessness.

About the role

The Outreach Case Manager’s role is a part-time position of 22 hours per week (days and hours to be negotiated between Tuesday and Friday). It is currently offered for a fixed term contract to 30 June 2023 and is dependent on funding past this date. The position is paid at the Social, Community, Home Care and Disability Services award Level 4 with salary sacrifice available. The Outreach Case Manager will work collaboratively and supportively within the Zillmere Family Accommodation Program team to deliver a new service that will allocate brokerage and provide high-quality case managed support to families who are experiencing homelessness or at imminent risk of homelessness.

As Outreach Case Manager, you will be accountable for providing brokerage and case management support to families who are homeless and living in temporary accommodation. This work will be conducted in accordance with policies and procedures, and under the direction of the Coordinator.

Your key responsibilities will include:

  • Provide an outreach service in collaboration with other service providers.
  • Meet with clients in varied and appropriate settings.
  • Develop case management plans in consultation with each client and according to their needs. Follow Case Management principles and standards.
  • Provide support to clients through strong advocacy, information dissemination, referrals, skill education and development, community connection in accordance with a client-centred approach.
  • Assess brokerage requests and assist in brokerage allocation.
  • Negotiate and coordinate with other service providers and Government and Non-Government organisations to ensure that supports and services provided to clients are appropriate and responsive to their needs.
  • Problem solve and creatively source solutions for temporary accommodation in the first instance and to facilitate access to sustainable longer term housing. Develop new relationships with accommodation providers.
  • Abide by Social Justice principles, Trauma-informed practice and right to confidentiality.
  • Record case notes using the Specialist Homelessness Information Platform (SHIP).
  • Assess the needs of applicants using the Queensland Homelessness Information Platform (QHIP) assessment tool and refer to vacancies. Other assessment tools and referral pathways may also be used.
  • Assist the coordinator with improving processes, procedures and with reporting as per funding requirements.

About you/Key Selection Criteria

To be successful in obtaining this position, applicants will meet the following criteria:

1. Tertiary qualifications in human services, community services or similar field.

2. In depth experience in housing support, tenancy sustainment and working with families experiencing homelessness.

3. Demonstrated abilities in case management practice and principles. Minimum 3 years’ experience.

3.1 Ability to identify the key elements of case management. Demonstrate how you would develop and implement a case plan for families.

3.2 Highly developed analytical & planning skills as they relate to case management, including the ability to assess risk, develop a safety plan and manage caseload.

3.3 Skills in needs identification, service planning, monitoring & evaluation.

3.4 Demonstrated knowledge of relevant community services as they relate to case management & care coordination.

4. Demonstrated ability to engage and work with adults and their accompanying children who present with a broad range of issues including mental health, domestic violence, child protection, poor living skills, debts.

5. Demonstrated ability to engage with people from diverse backgrounds (cultural, language, age, experiences).

6. Demonstrated ability to work effectively in high pressure fast paced environment. Demonstrated excellent time management skills.

7. Demonstrated problem solving and creative thinking skills in the workplace.

8. Must possess excellent written and oral communication skills.

9. High level knowledge of computer programs including Microsoft Office Suite and Homelessness specific information systems QHIP and SHIP.

10. Demonstrated ability to work collaboratively in a team environment and be accountable as an individual worker.

11. Valid Queensland Driver’s License and vehicle.

12. Working with Children’s card (Blue Card) or eligible to get one as card is required to commence employment.
