As a Case Manager you will be part of our Melbourne team and support people with a range of complex high physical needs to achieve their goals and live an independent life. Reporting to Operations Manager you will take ownership of your own group of participants within Northern Suburbs of Melbourne Metro and manage all client referrals, on-boarding and engagement throughout their journey with The Nextt Group.

About the role:

Along with extensive knowledge in Disability Industry with focus on Complex High Physical Supports and high level of stakeholder management, you will also:

  • Ensure all new participant enquiries are captured and documented for their region
  • Ensure participant needs, preferences for support workers are captured and briefed into scheduling and people and culture teams
  • Implement care plans in line with NDIS and participant needs
  • Facilitate ongoing support to our NDIS participants by exceptional customer experience, engagement and support in the achievement of their goals
  • Build relationships with clients, their families, external providers and internal stakeholders
  • Assist participants to maximise the benefits of their package/plan in an acceptable timeframe and within allocated funding
  • Capture and maintain client information across various management systems
  • Implement and manage critical incident reporting, compliance and client retention
  • Support the regional management team with the implementation of strategic business planning

About you:

To be considered for this role, you will have:

  • Tertiary qualifications in Disability, Allied Health, Physiotherapy, Mental Health or related
  • Minimum 2 years of experience in Client Interaction role in Disability, Health or related
  • Demonstrated experience in customer service and in managing and implementing active supports and strength-based recovery approaches to support participants to have a meaningful life
  • Experience of the intake / assessment function, planning and service delivery within NDIS Framework is a MUST
  • Ability to prioritise, work under pressure and meet deadlines
  • Work collaboratively with internal and external stakeholders
