The Role:

You will join a high performing and passionate team dedicated to the best possible outcomes for children and young people! This rewarding opportunity will see you responsible for the effective case management in ONCALL’s Youth Services, including Residential Care, TCP’s and In-Home Support programs.

Reporting to the Senior Case Manager, your day to day responsibilities will include:

  • Deliver the case contracting for Residential Care and Targeted Care Packages (TCP's);
  • Ensure assessment planning, facilitation, care-coordination, evaluation, advocacy and support of TCP and IHS for children, young people and families meets individual and family needs and achieves cost-effective outcomes;
  • Undertake site visits will all clients and guardians with TCP;
  • Attend all court hearings related to the child or young person;
  • Complete and lodge all quarterly and court reports as required;
  • Attend and facilitate all Care Team Meetings;
  • Ensure all documentation and systems are updated in line with quality and legislative standards and ONCALL’s policies and procedures;
  • Build and maintain effective working relationships with all key stakeholders both internal and external;
  • Report monthly on activity, outcomes and areas for improvement.

To be successful in the role you will demonstrate:

  • Tertiary qualification in Social Work, Social Welfare, Education, Psychology or related area;
  • Demonstrated experience in child protection, case management, child welfare practice or a related field;
  • Successful coordination and placement in OOHC specifically with Residential Care and/or Targeted Care Packages
  • Understanding of DFFH standards, CYF Act 2005, Children and Young Persons Act 1989, Child Safety Standards and best interest principles;
  • Knowledge of trauma informed practices, case management and the CYF sector;
  • Working knowledge of CRIS, CRISSP or other relevant databases;
  • Knowledge of child protection and issues in relation to children in risk and an understanding of relevant theoretical frameworks;
  • Demonstrated knowledge of the issues facing young people who need and rely on OOHC services;
  • Demonstrated ability to facilitate good working relationships with all services, clients and their families and significant others.
  • Be able to participate in an after-hours roster.
