Client Intake Lead - Civic Disability Services

About this Role:

Reporting to the General Manager Brand Experience, and working as part of the Customer Experience Team this role is critical in ensuring that all our clients experience an exceptional customer experience at every stage of their journey from being onboarded as a new client right through to liaising with various key stakeholders, both internal and external to the business, in order to achieve positive client outcomes.

Key Responsibilities:

  • Conducting functional behavioural assessments
  • Collaborating and communicating with families and carers to better understand the needs and preferences of prospective clients
  • Analysing, collating, and presenting data to the Clinical & Service Team on recommended accommodation locations
  • Plan, coordinate and manage the onboarding process of the client to minimise the impact on them and ensure a satisfactory onboarding process

What are we looking for:

To be successful for this position, it is essential that you have a passion for facilitating excellence in customer service, together with:

  • Tertiary Qualifications (Degree or Diploma) in Social Work / Psychology / Disability / Occupational Therapy or similar
  • Proven experience in functional behavioural assessments and writing plans to manage complex customer journeys, with the ability to bring together numerous stakeholders
  • Proven ability to relate to customers in a relatively short period of time, understand their needs and identify possible services to suit these needs.
  • Demonstrated experience utilising a customer relationship management (CRM) system
  • Experience with NDIS plans and funding
  • Outstanding communication skills and the ability to build and maintain relationships across various stakeholders (e.g. various business units, customers and their families)
  • Proven ability in achieving results working in a fast paced and dynamic environment whilst able to respond to multiple deadlines
  • Meticulous record keepings skills
  • Passion / interest in the community or human services sector
