We have an exciting opportunity which has arisen in our TFCO team - the position being Case Co-ordinator / Liaison worker

Treatment Foster Care Oregon (TFCO) is an evidence-based program providing an alternative to residential care supporting reunification outcomes for children and young people aged 7-11.

As OzChild Liaison Worker/Case Coordinator you work alongside and in collaboration with the Treatment Foster Care Oregon team under the guidance and support of a program supervisor. Your role as part of that team is predominantly case management. Identity and promote goals associated with health, wellbeing, leisure and life skills for young people. This role is designed to help facilitate access to wider services and maintain case plans as well as maintain and oversee effective management of all cases and provide a broad level of support to the clinical team.

What you will do

  • Provide information and comment to the team as to whether case plans goals are being met
  • Make sure all information regarding the cases is up to date and recorded
  • Support carer recruitment activities
  • Measure and record outcomes
  • Assist with the collation, dissemination and administration for the various behaviour charts for the young people
  • Provide Case Management to Children in the TFCO Program and maintain access to other services
  • Monitor and review individual strength-based care plans

Who we are looking for

  • Tertiary qualifications in Community Services, Child Protection, Social Work, or related field
  • Experience with assessing and supporting foster carers
  • Ability to juggle different tasks
  • Demonstrated experience in the out-of-home-care sector
  • Excellent verbal and written communication skills, facilitation, and conflict resolution skills
  • Strong computer skills including database entry, word processing, and the Microsoft Office suite of programs
  • Proven ability to manage multiple deadlines
  • Demonstrated experience working with a variety of services, agencies, and community groups
