Professional Care Supports is a registered NDIS Provider seeking to make the journey for NDIS participants a little easier. Our background is in Psychology and Social Work so we are in a unique position to provide many levels of service provision including support workers; Coordination of Supports Levels 2 & 3; etc. Developing care plans that are individually tailored to include a participant’s goals and maintain a regular check in that these remain so. As a small independent agency, we can devote the time necessary to ensure best outcomes for our clientele. We are located in Newcastle and Central Coast and actively seek positive outcomes for individuals by linking them to mainstream services, Allied Health, Employment Options and whatever other requests they may have.
We are seeking an experienced Coordinator of Supports with NDIS experience who engages with participants and their informal support networks and builds connections with providers, local community services and NDIS contacts.
Supports participants to understand their NDIS plan and plan funding, and to design their support framework within available plan funds to their achieve goals.
Coordinates participants to connect and engage with providers, to access supports, and builds participant capacity.
Links participants to mainstream services and gathers, prepares evidence for plan reviews.
Has a clear understanding of NDIS protocols and adheres to current best practice including legislative requirements
You will have good time management and project management skills; Ability to self manage multiple tasks and often competing ones; a strong level administration/technological use.
Tertiary qualifications in mental health and/or disability would be desirable.
