This is an office based senior role within the interdisciplinary team responsible for the management of client related phone calls and issues during business hours, and the effective allocation of staffing resources.

Peninsula Home Hospice is an accredited, not for profit community palliative care organisation providing specialist health care and practical support to people living at home with a life limiting illness and their families and friends. PHH supports people who live within the Mornington Peninsula Shire, City of Frankston and City of Kingston (south of Mordialloc Creek).

position objective

Working within the Palliative Care Clinical Team, the Liaison Nurse is accountable for providing high level customer service to clients and carers and operational support to the Clinical Leads.

The key Selection Criteria is as follows:

  • Registered Nurse Division 1 with current certification from the Australian Health Practitioner Regulation Agency (AHPRA)
  • Advanced knowledge and skills in the principles and practices of palliative care
  • Previous experience in community palliative care or similar
  • Excellent and demonstrable experience in telephone clinical assessment and support
  • Excellent time management and organisational skills
  • Well -developed interpersonal written, verbal and negotiation skills
  • Excellent and demonstrable computer skills
  • Post graduate qualification or working towards same, in palliative care, oncology or community nursing
