Youth Development Specialist / Case Manager

Phoenix House provides case management to young people (aged 12-24) who may require additional support to maximise their potential and achieve client centred goals. We adopt a strengths based responsive approach to case management which is underpinned by trauma informed care and evidence based practices which makes our case management more meaningful through therapeutic interventions.

We provide educational programs for young people who may have disengaged from mainstream education, or require additional support to meet their educational objectives. Our flexible and innovative approach to learning “engages the disengaged” which is closely supported by our case management team.

Phoenix House Youth Services employs a multidisciplinary team of professional, enthusiastic and passionate staff. We have developed a fully integrated, wrap around service model based on our three key objectives: LEARN READY, WORK READY, LIFE READY.

  • Direct report to the Client Care Manager
  • Adhere to PHYS policies and procedures
  • Ability to work autonomously and be a part of a cohesive team
  • Liaise with PHYS program managers to provide client centred case management support to young people aged 12 - 24.
  • Adopt a non-judgemental approach, promoting equality, diversity, and anti discriminatory practice.
  • Ensure all information is recorded in a factual, concise manner in the relevant data capturing systems. E.g CRM and CIMS
  • Develop care plans with young people to monitor and review client centred goals.
  • Participate in all PHYS protocols including client referral, intake, assessments, reviews and evaluations.
  • Report any incidents, concerns or risks of harm to the relevant program manager
  • Develop partnerships with relevant services, agencies and organisations specialising in services for youth and the community.
  • Identify gaps in service delivery to vulnerable youth and explore ways of addressing those needs within PHYS, the local community and within the sector.
  • Maintain a service of quality to clients of PHYS and where appropriate their families, working to identify life goals and strategies to achieve these.

Essential criteria:

  • Diploma or Bachelor degree in youth work, social work, community development or psychology.
  • Minimum 3 years case management experience working with trauma affected, marginalised or disengaged young people
  • Demonstrated ability to understand adolescent development through complex trauma, mental health, drug and alcohol and associated behaviours.
  • Excellent communication and interpersonal skills to engage, disengaged young people, their families and carers.
  • Ability to respond appropriately to incidents and knowledge of mandatory reporting requirements.
  • Demonstrated ability to deliver and evaluate youth specific group work programs.
  • Demonstrated ability to work from a strength based holistic framework when conducting assessment and deliver services.
  • Ability to maintain administration and information systems accurately and efficiently.
  • Capacity to work with community partners to provide a comprehensive service delivery addressing identified needs.
  • Willingness to engage in in-house supervision and professional development opportunities.
  • Current driving licence.
  • A valid WWCC.
  • A valid police check

Desirable Criteria:

  • Knowledge of young people experiencing homelessness.
  • Experience in running youth development programs and capacity building initiatives with young people.
  • Engage in regular supervision as part of ongoing support and participate in identified training and development needs.
  • Work towards completing identified KPI’s and participate in performance appraisals and reviews
  • Work in accordance with the agreed chart of delegations of authority and expenditure.
  • Attend internal and external meetings as required.
  • Participate in and contribute to Health and Safety awareness and improvements.
  • Comply with emergency evacuation procedures.
  • Report all incidents/injuries and potential hazards in a timely manner.
  • Assist to maintain appropriate and efficient accountability, administration, and information systems.
  • Assist to maintain a comprehensive system for recording and securely storing client records and client related information.
  • Assist to maintain appropriate data storage systems related to the operational aspects of the organisation.
  • Ensure that all data and record keeping systems are utilised accurately and effectively.
  • Attend to incoming correspondence, telephone calls and other inquiries in a timely manner.
  • Comply with and adhere to all relevant legislation including Complaints, Appeals and Monitoring Act, NSW Anti-Discrimination Act, Federal Human Rights Legislation, Work Health and Safety Act.
  • Comply with and adhere to Phoenix House Youth Services Inc. policies and procedures.
  • Perform other duties as negotiated, deemed reasonable and delegated by the Client Care Manager and/or Executive Director.
