PRONIA is seeking a suitable applicant for the Case Manager Position on a full-time basis located in Oakleigh and may be required to travel to other sites across Victoria. Reporting to the HCP Coordinator, you will be responsible for providing support to our Home Care Package clients, monitor care recipient needs, seek opportunities for growth in the Home Care Packages program and undertake additional tasks in line with the overall position relating to PRONIA.

The successful applicant will have, amongst other attributes, appropriate tertiary qualifications preferably in Social Work, nursing or allied health fields, minimum 1-year experience in community aged care or case management, awareness of issues for Greek speaking carers and the ability to work cooperatively and autonomously within a team environment.

Proficiency in verbal and written Greek and English, coupled with excellent time management skills is essential. Proficient use of Microsoft Office such as Word, Excel, Access, Internet and E-mail are also required.

Mandatory:

  • Current Driver License.
  • Access to own vehicle.
  • Undergo Criminal History Record Check at the required intervals.
  • Own IT infrastructure at home to enable work from home capabilities during periods of lockdown or where directed by the business.
  • Fully immunised against COVID-19.
