We have an amazing opportunity for a passionate, confident, and aspirational Care Coordinator to join our enthusiastic team. This role will enable you to utilise your skills in the coordination of disability/mental health support services across Darwin, Ideally you will be a resilient, committed individual with a sound understanding around the provision of supports in supported independent living arrangements as well as other community and in home support services. About the role
You will be tasked with mentoring teams and managing the day-to-day service provision of our services, you will ensure a high standard of service provision is maintained. You will also use your well-developed communication skills to engage with multiple stakeholders to work in a collaborative approach in providing the highest standard of care to the people we support.
Responsibilities:
- Mentoring, coaching, and managing your teams by regular engagement, support, and development
- Managing performance within your teams
- Manage client intake, assessment, and reviews
- Liaison with NDIS planners, LACs, Govt agencies and various other stakeholders in relation to service delivery and outcomes
- Developing, assessing and reviewing the Participants’ care plans
- Regular auditing of services to ensure all sites are working in accordance with Purple Support policies and procedures
- Bridging with multiple stakeholders to ensure effective communication, responsiveness and an overall collaborative approach is achieved
- Be responsive to NDIS participants and their stakeholders to ensure needs are being met
- Take a person-centred approach in assisting teams to support service users to strive toward their NDIS goals
- Complete required reporting and documentation within the appropriate timeframes
- Provide after-hours support to your services when required
What You Need:
- Cert III/Diploma in Individual Support or equivalent qualifications
- Minimum 2 years experience managing teams in a supported independent living arrangements or similar arrangement
- Minimum 4 years experience in the disability or mental health sector
- Experience working with people with complex mental health needs highly desired
- Qualifications and experience relating to the disability/mental health sector or similar fields
- Efficient understanding & experience in managing disability or mental health support services
- Experience leading a multidisciplinary and collaborative team
- Experience working within allocated budgets, monitoring expenditure and record keeping.
- Ability to build rapport with participants, guardians, key service providers and stakeholders
- Proven ability to mentor and support teams
- NDIS Workers Screening Check
