Emerging Futures Collaborative (EFC) is a ministry established by a collaborative of Catholic Religious Institutes in Australia to provide administration and support services to Religious Institutes.

EFC seeks to appoint a Health and Wellbeing Coordinator to manage health and wellbeing support care services to members of religious institutions in Sydney. The initial client service responsibility is focused on Sydney-resident members (living independently) of the Presentation Sisters, Wagga Wagga but may change subject to other EFC service partner needs, or re-organisation of EFC service resources over time.

The Presentation Sisters Wagga Wagga have 18 sisters living in Sydney’s inner west and the Macarthur area.

The Role

Reporting to the Service Delivery Manager, EFC, the role of Health and Wellbeing Coordinator will join the small EFC Team and be responsible for ensuring every Sister living independently, receives quality health and ageing care and support which is planned, delivered, monitored and reviewed in a manner that reflects best practice.

You will ensure every sister in need of assisted living support or clinical care receives a full suite of quality clinical and care services provided through a mix of government and Congregation funded services. This will include comprehensive in-home care and support, and appropriate clinical support.

You will also ensure that each Sisters’ health and wellbeing activities are managed in an accountable, efficient and cost-effective manner which is underpinned by a management system (comprising a comprehensive suite of policies and procedures) which reflects generally accepted best practice and meets all statutory requirements related to safety, compliance and care.

This role is a permanent, part-time position working the equivalent of 3 days per week flexibly across the week as needs require. This role will be based at the Presentation Sisters Wagga Wagga Sydney office in Berala, however you will mostly be out in the local area with individual sisters.

Key Requirements

Your experience in case management or care coordination in the aged care sector, as well as your understanding of government funded services and how to make best use of them will be key to this role.

Specifically, you will ideally possess the following core capabilities:

  • Qualifications as a Registered Nurse with at least 5 years post registration experience in a hospital, community or aged care setting
  • Ability to support the sisters through various transitions which might include navigating community services and systems — e.g., sourcing care packages
  • Ability to coordinate clinical practice and care to meet each sisters’ requirements
  • Experience of implementing and updating systems and processes for service delivery
  • Ability to apply regulations/legislation in relation to clinical risk and quality improvement processes
  • Strong communication, listening and interpersonal skills and experience, together with the ability to advocate and influence stakeholders to achieve desired outcomes
  • A personal desire to support elderly people to live valued and meaningful lives
  • Appreciation of and commitment to supporting the ethos, culture, mission and charism of the Presentation Sisters

The successful candidate will be required to undertake a Nationally Coordinated Criminal History Check and Working With Children Check. You will need to be fully vaccinated against Covid-19 and maintain annual influenza vaccinations to work with the more vulnerable sisters.
