Every day of the year, Sacred Heart Mission (SHM) assist hundreds of people who are experiencing homelessness or living in poverty to find shelter, food, care and support. We are in this for the long haul - we are committed to making a sustainable difference in people’s lives. Everyone is welcome at our table.
The Wellness Place provides support to at least 150 clients each month through a range of allied health therapies and skills development activities.
The Community Inclusion Worker-Cool Change is a new part time role within The Wellness Place (WP). In this role, you will work across Engagement Hubs and Wellness Place to build relationships and enhance participation and quality outcomes for Hub clients.
Your typical day will include individual, time limited support to up to 6 clients in order to facilitate their participation in groups or other Wellness Place or community activities.
You will provide assertive engagement to more complex engagement hub users living with homelessness, mental illness, substance use and social isolation. You will also specifically target Hub clients who are not help seeking and may be reluctant to try new things.
