Every day of the year, Sacred Heart Mission assist hundreds of people who are experiencing homelessness or living in poverty to find shelter, food, care and support. The GreenLight Supportive Housing Program is run in partnership with VincentCare, and Salvation Army. We support people who have experienced chronic homelessness including rough sleeping to establish a home and community connection, provide specific interventions to sustain tenancies, and will support underlying health and other issues that may impact on people staying housed
The role is one of two Health and Wellbeing Worker/s roles in the GreenLight program. The role will be located on GreenLight teams based across the inner Melbourne area by SHM, VincentCare, and Salvation Army and will use assertive engagement and outreach as the primary service delivery mode.
This part time role (30.4 hours per week) will provide health and wellbeing support and expertise in areas such as mental health, AOD, general health and NDIS to the GreenLight Program staff and clients through the provision of: primary consultation, staff training and capacity building, and systems navigation, and lead for service coordination in relation to multi-agency mental health response/ services.
