Sacred Heart Mission's Journey to Social Inclusion (J2SI) program is an evidence-based intervention designed to break the cycle of chronic homelessness. We know the current service system is not effective and we're taking a different approach in our goal to end homelessness.
You will be based at Sacred Heart Mission in St. Kilda. You will work with a multidisciplinary team to assist people to settle into housing, stay housed, and improve their quality of life. The role will see you working in the community and in people’s homes as you assist them to develop goals and navigate services that meet their needs to stay housed. While every day is different, a typical day might see you on outreach, visiting one of your many participants and supporting them to settle into their housing and connect to their community. You may then return to the office for a check-in or group reflective practice with your colleagues.
