One Door Mental Health is a leading for purpose service provider for those living with mental illness, their families and carers. The team operate locally as self-organised teams - called Recovery 2gether teams. Team members are collectively responsible and accountable for:

  • Working with the people we support (e.g. clients, consumers, family, and carers) and
  • Undertaking key functions (eg. Recruitment and onboard, Quality and Safety etc) by a nominated Function lead

Team members work collaboratively with other team members to facilitate the effective operation of the team supported by an Operations Lead/Recovery 2gether Coach.

The Program

The Bilingual Hospital to Home (H2H) supports people living with mental illness, and people who have self-harmed or attempted suicide; for up to 12 weeks following discharge from hospital. We help people to connect, to live well in their community.

The role

The Bilingual Mental Health Worker is responsible for building relationships and capacity within the Western Sydney Community to strengthen co-ordination, referral pathways and connections between GPs, Allied Health Providers, Government Agencies, Health Services and Community Services to improve support for individuals from culturally and linguistically diverse populations who are impacted by severe and persistent mental health conditions.

The key responsibilities include:

  • Work with the people we support in the mental health units to help plan their discharge with the mental health team, define their goals and needs for the first 6 - 12 weeks after discharge, and then support the individual in the process of implementing the plan and linking to relevant services and supports
  • Develop relationships and work closely with Local Health District Community Mental Health teams Hospitals (including Transcultural Mental Health Mental Health staff, Settlement Services, Diversity Services and other relevant services in the Western Sydney to accept and make referrals
  • Liaise with unit staff, GP, Allied Health and relevant services to support the individual to access services away from the hospital setting and within their local community in the transition period from Hospital to Home
  • Build collaborative relationships with key culturally and linguistically diverse community leaders and members to support individuals, families, and carers in those communities to improve access and connections to appropriate support
  • Provide Community and Consumer lifestyle, capacity building, and education Groups and promote weekly social connection and ward for consumers in the program

What we are looking for

  • Relevant tertiary qualifications to Certificate IV in mental health, peer work or related field, and/or two years of experience in mental health related
  • Bilingual - fluent in relevant language/s of the culturally and linguistically diverse communities in Western
  • An understanding of the recovery- based approach to assisting people with severe and persistent mental illness
  • Knowledge of the NSW mental health system and local services
  • Ability to communicate effectively in all interactions with all stakeholders
  • Strong relationship building skills to navigate, collaborate and develop partnerships with the NSW mental health system and local
  • Fluent in Arabic, Farsi and Vietnamese.
