The opportunity

We have an exciting opportunity for a Community Care Leader to join our team in Kalgoorlie on a full-time basis. Whilst reporting directly to the Community Care Manager, you will support staff, clients, carers and their support networks. You will be a qualified health professional who will ensure that services are delivered effectively and that care plan objectives are being achieved. 

Key responsibilities

  • Undertake comprehensive, holistic assessments of clients at admission and ongoing care as required
  • Formulate and maintain care plans and required documentation in consultation with the client, carer and/or family
  • Work in a collaborative partnership with other community care services to support any shared clients and their carer's.
  • Collaborate with, and seek advice and assistance from other relevant Health Care Professionals internally and externally
  • Monitor and develop employee performance through regular performance appraisals
  • Lead and manage employees to ensure best practice is followed and compliant
  • Manage and monitor compliance to policies, procedures, contracts, accreditation and legislative obligations to ensure quality of services and work practices in line with established clinical standards.
  • Monitor and co-ordinate resources (equipment, direct care staff, etc) and provide support and education to ensure service delivery is in accordance with the plan of care.
  • Oversee programs of work within area of specialisation and location of role such as Social Centres and associated activities.
  • Deliver client care in accordance with current professional and ethical standards, evidence-based practice, relevant legislation and common law
  • Provide specialist education, support and demonstrations to internal and external parties to ensure quality service delivery.
  • Build relationships and network with internal and external parties to promote Silver Chain services and identify opportunities for consumer involvement, growth and new business with the local community

About you

In addition to your relevant qualification in an Allied Health or Nursing discipline, with current AHPRA registration (or equivalent) you will possess: 

  • Demonstrated clinical experience within the applicable scope of practice.
  • Demonstrated experience in leading a team to achieve outcomes.
  • Demonstrated knowledge of service provision, allocation and compliance with contractual obligations to ensure quality service provision.
  • Well-developed communication (written, verbal and listening) and interpersonal skills to develop effective relationships, promote services and resolve client/staff issues.
  • Demonstrated decision making and problem-solving skills to innovatively manage the provision of quality care.
  • A demonstrated empathetic and flexible approach to manage the sensitivities of client and employee issues.
  • Demonstrated computer literacy in word processing, spread sheets, and calendar management software to prepare correspondence and effective documentation.
  • The ability to work autonomously with a team focus to contribute to business unit and organisational objectives.
  • A demonstrated understanding of health care within a community environment to co-ordinate quality service delivery.
  • Demonstrated time management and organisational skills to effectively plan and prioritise work tasks.
  • Demonstrated knowledge of manual handling principles with the ability to train others.
  • Senior First Aid Certificate is essential
