About the role

Concord Repatriation General Hospital has a well-earned reputation for excellence in healthcare for over 70 years. Building on its proud heritage of caring for the Veteran community, the hospital now services the local communities of Concord, Strathfield, Burwood and beyond.

Applications are sought from experienced Registered Nurses to join the Access Care Team (ACT) to support the delivery of intake services for Aged Health Chronic Care and Rehabilitation services; and Sydney District Nursing. ACT also provides telephone triage and telehealth services to support residential aged care facilities in the care and management of its residents.

The position will require you to have excellent organisational and clinical skills with the ability to assess and triage calls and referrals based on clinical need. In addition, a high level proficiency in the use of computer software applications is essential for this position.
