The SMRPCC is an alliance of palliative care services in the Southern Metro Region of Melbourne with the aim to collaboratively implement the Victorian End of Life and Palliative Care framework in the region. The consortium is committed to assisting RACFs in the region to improve the palliative approach.

Peninsula Home Hospice is the fundholder for the consortium and employs consortium staff.

Position Context

There are approximately 160 RACFs in the region. The consortium has been assisting these facilities to establish systems to improve the palliative approach. Coordination of external initiatives, information sharing and direct support to interested facilities are the main components of the work.

Position Purpose

The position purpose is to enable RACFs in the Southern Metro Region to better implement the palliative approach.

Key Areas of Responsibility

Establishing and maintaining contact with managers and other relevant staff in RACFs

• Communicate relevant federal and state initiatives in the space of RACFs and palliative care (ELDAC, Greater Choices at home, PEPA, etc)
• Supporting the community palliative care services in the engagement with RACFs
• Implementing the DRFA project, including support initiatives
• Collect and maintain relevant data
• Evaluate project outcomes
• Maintain regular communication with other consortium aged care project workers
• Maintain regular communication with the regional Primary Health Network

Selection Criteria

• Nursing background (RN Div1)
• Significant experience in palliative care
• Good understanding of the aged care system
• Excellent interpersonal and stakeholder management skills
• Demonstrated experience in preparing and delivering presentations in a variety of different formats e.g., face to face, webinar, via virtual technology
