Team Leader - Out of Hospital Care - Maitland

  • Full time - 38 hours per week

Seeking a passionate professional to lead a new small team of Care Coordinators across Lower Hunter and in Broken Hill. The team will provide access to Out of Hospital Care packages and supports to assist eligible people obtain the after hospital care they need. Bringing your experience in case management across client assessments and planning activities in health, aged care or disability care settings, you will coordinate point of contact for these customers and community, and conduct assessments in hospital and home settings.

This is a new hands-on role in a dynamic environment which is going through a significant period of growth and change.
