Status:

  • Part-time to Full-time (up to 37.5 hrs per week)
  • Contract position – funding secured until 30 June 2023

Working Location:

  • Gawler Barossa region. Working from our Nuriootpa office and outreaching to Gawler and the Barossa.

We are seeking passionate and enthusiastic Mental Health Nurses and other appropriately qualified mental health professionals to join our dynamic, multi-disciplinary and growing mental health team to play a crucial role in the delivery of Clinical Care Coordination.

Clinical Care Planners play a key role in helping to provide care coordination for people with severe and complex mental illness managed by GPs and Psychiatrists in the community. Working within a recovery orientated framework, a joint care plan is developed with the individual and their key personal and professional supports to improve physical health and support effective management of their mental health symptoms and avoid unnecessary hospitalisation. Support is targeted, complementing and enhancing existing GP, Psychiatrist and allied mental health services.
