Sonder’s In-Home Withdrawal Service provides comprehensive, holistic and coordinated support by a multidisciplinary team of In-Home Withdrawal Senior AOD Coordinator, In-Home Withdrawal Nurses, In-Home Withdrawal Clinical Workers, and AOD Peer Support Workers, working alongside GPs. To ensure the best possible outcome, the withdrawal management process is supported by pre withdrawal care coordination, assessment, preparation, and brief intervention (Pre-Care) and post withdrawal support, relapse prevention, and care coordination (After-Care). In-Home Withdrawal Nurse will work as part of a multidisciplinary AOD team to deliver focused evidence based treatment options for individuals experiencing substance dependence and wishing to undergo in-home withdrawal.

Reporting directly to the AOD Services Team Leader, your responsibilities will be:

  • Provide high quality, evidence based services to assist with the clinical assessment and medical management and supervision of clients undergoing in-home withdrawal.
  • Provide ongoing clinical monitoring and observation of clients, including: close liaison with GPs for assessment of medical needs during in-home withdrawal, the monitoring of withdrawal symptoms, mental health screening and assessment and documentation of progress
  • Provide information, education, and practical assistance to clients and families/carers to help them to manage the in-home withdrawal process
  • Psychoeducation and behavioural therapies as appropriate

Experience requirements:

  • A minimum 2 years supervised experience in the area of clinical AOD service delivery.
  • A minimum 2 years relevant professional development and or supervision.
  • Demonstrated experience working within a fast-paced environment.

Qualification requirements:

  • Registered Nurse registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australian and who holds, or who is eligible to hold a current practicing certificate
  • Current National Police Check
  • Current (or willing to obtain) WWCC check
