SSI has partnered with the National Disability Insurance Agency (NDIA) to deliver services as a Local Area Coordinator (LAC) in NSW. We support people aged 7 and over to access the National Disability Insurance Scheme (NDIS) and other mainstream services in the community. We want to ensure our workforce reflects the participants we work with to help meet their needs, and are seeking candidates with disability to bring their own experience and insight to this role.

The LAC Planner will work with the participant (and where appropriate their family and other informal supports) to determine what reasonable and necessary funded supports and services will assist the participant to achieve their goals. They will gather the relevant information, to build and develop the plan for NDIA approval. They will also work with participants to make changes through the plan review process which covers scheduled and unscheduled reviews.

As an LAC Planner, you will have great opportunities to influence the outcomes of people with disability and your ability to communicate with both participants, their families and carers will be extremely valuable. SSI will provide training to ensure your success in this role.

Duties and responsibilities:

  • Gather information to support Participants to make informed decisions regarding their plan, based on reasonable and necessary criteria.
  • Work closely with participants to identify support needs, goals and aspirations, to enable better individual outcomes.
  • Scheduling appointments and facilitating planning conversations.
  • Providing relevant in-language information and advice on the NDIS, and disability/mainstream service.
  • Linking participants to mainstream services and building the capacity of individuals to access relevant supports.

ABOUT YOU

You possess proven interpersonal and communication skills with the ability to build effective relationships and liaise across all levels both internally, externally and with people from diverse backgrounds including Aboriginal and Torres Strait Islander and CALD communities. You have demonstrated ability to work with limited supervision, manage a variety of tasks and display personal drive and integrity. All LAC Planners need to have the willingness to travel and work flexible hours and in a range of locations across areas and regions as required.

  • 5+ Years' experience working with people with vulnerable people with complex needs.
  • Relevant qualifications in human services or community development and/or Demonstrated experience working in the human services sector (e.g. disability, child services, allied health, social work, community development, education) and/or relevant qualifications or training in social inclusion, community development and engagement or person-centred thinking and planning.
  • Previous experience of working in a fast-paced, KPI and outcomes driven environment.
  • Ability to use and navigate a variety of complex reporting mechanisms including client databases and Microsoft Excel.
  • Strong written and verbal communication skills.
  • Willingness to learn new skills and procedures required of the role.
  • Awareness of the impact of disability upon individuals, families, carer and the community and is empathetic with a genuine desire to help people with disabilities gain a better life.
  • Awareness and recognition of cultural diversity, including Aboriginal and Torres Strait Islander and CALD communities
  • Ability and willingness to travel regularly across the SWS and Sydney Districts to attend Participant meetings etc.
