About the Program:

Settlement Services International (SSI) has recently been advised of our successful tender to deliver the National Disability Insurance Scheme Partners in the Community Program (PITC), in the for the next three years. The program provides for Local Area Coordination (LAC) services to assist participants in developing or reviewing support plans as well as extensive work in the community to promote inclusion and opportunities for people to access mainstream service.

About the Role:

As a Local Area Coordinator (LAC) at SSI, you will be the heartbeat of the NDIA in your local community, working with people with disability and their families to access and navigate the National Disability Insurance Scheme. Duties and Responsibilities:

  • Supporting participants in the development of goal-based, individualised plans which support their strengths, skills, and interest
  • Guiding participants to identify what current and future supports they require
  • Scheduling appointments for the LAC team and NDIS planning meeting
  • Providing information to participants and their representatives to identify their goals and aspirations
  • Guiding and assisting people with disability to build capacity and explore options for self-direction, self-management, self-advocacy
  • Building and maintaining positive partnerships with the wider community to refer participants to a diverse range of service providers

About you:

You possess proven interpersonal and communication skills with the ability to build effective relationships and liaise across all levels both internally, externally and with people from diverse backgrounds including Aboriginal and Torres Strait Islander and CALD communities. You have demonstrated ability to work with limited supervision, manage a variety of tasks and display personal drive and integrity. All LAC Planners need to have the willingness to travel and work flexible hours and in a range of locations across areas and regions as required.

  • Bachelor equivalent qualification in relevant or related field e.g. Psychology, Social Work or Allied Health or Qualification in disability/community services with equivalent experience working with people with complex needs
  • Demonstrated experience in work closely with participants to identify what current and future supports are required to make progress with a persons goals and aspirations.
  • Proven experience conducting planning conversations with participants to create plans to support the goal of living an ordinary life
  • Ability to provide relevant in-language information, advice on Disability and NDIS
