Stepping Stone, established in 1994, is an incorporated, non-government, not-for-profit organisation, dedicated to assisting people with a mental illness.

The organisation is based on the Clubhouse International model (www.clubhouse-intl.org) and has been accredited by Clubhouse International since 1997. Stepping Stone abides by the International Standards for Clubhouse Programs.

Stepping Stone's mission is to assist adults with a mental illness to develop the skills and confidence necessary to live satisfying and productive lives in the community. This assistance is provided in a psychosocial program designed to empower and support its members.

Stepping Stone provides a non-institutional setting where adults with a mental illness work side by side with staff as they work to rebuild their confidence, stamina, concentration, social and vocational skills. It is a voluntary program whose participants are members, not patients or clients.

The organisation believes that every member has a contribution to make and that recovery is possible. Stepping Stone relies on members' talents, skills and strengths in order to function.

About the Opportunity

Stepping Stone Clubhouse Inc. is seeking a NDIS Support Coordinator for an initial 6 month position. Stepping Stone is based in Coorparoo however the role allows for great flexibility and the opportunity to regularly work offsite.

The successful candidates will be rewarded with a competitive starting salary of $72,360, in line with SCHADS Level 4.1, plus superannuation, with the opportunity to access generous salary sacrifice options.

As a Support Coordinator, you will provide varying levels of coordination of supports for adults with mental illness. Considering an individual’s circumstance and level of complexity, the role actively supports participants to coordinate and manage their NDIS (National Disability Insurance Scheme) plan to enable greater independence across all domains of an individual’s life.

Your key responsibilities will include:

  • Welcome and engage the member/external referral and any supports to ensure they are fully aware of the role of support coordination in assisting them with the effective implementation of their NDIS plan.
  • Ensure the member/external referral is provided with a service agreement prior to providing any support coordination, is fully informed about its contents and signs it as part of the engagement process.
  • Act as a central reference point for the member/external referral and their family/carers in the coordination of the persons NDIS plan.
  • In collaboration with the member/external support, identify key recovery goals noted in their NDIS plan and the relevant services to support these goals.
  • Co-ordinate a broad range of service providers and ensure a team approach to addressing the member’s/external referral’s support needs.
  • Ensure all services engaged in the support of the member have a shared understanding of the elements of the NDIS Plan and that all service providers work in a coordinated and integrated way.
  • Coordinate the ongoing review of the effectiveness of the supports on an ongoing basis and provide detailed progress reports to the NDIA as required.
  • Meet all data entry, administrative and reporting requirements for NDIS and other funding bodies
  • Submit reports to NDIA within expected time frames.
  • Ensure Policy and Procedures are adhered to and work in line with Stepping Stone’s values.
  • Maintain up to date knowledge of NDIS systems, practices and legislation.
  • Maintain up to date knowledge of community services and organisation’s participants can utilise.

In line with the Clubhouse model, the role will also include general support duties, such as:

  • Promoting program development in accordance with the Clubhouse International philosophy as outlined in the International Standards;
  • Facilitating the work of the unit in conjunction with the Director and your direct Supervisor;
  • Work side by side with members to ensure that they are engaged and supported in the unit with the aim of increasing members' self esteem, concentration, stamina and social skills;
  • Developing and maintain meaningful work opportunities in the unit for members;
  • Work with members to assist them to identify their goals, and provide individual support such as employment, education, housing, advocacy, and assistance to access external services.
  • Assisting in the outreach program providing phone, email, mail, home and hospital visits;
  • Presenting to community groups regarding mental illness and the Clubhouse model; and
  • Participating in education and colleague training.
