For 20+ years St John has provided quality home support services to the Queensland to access their community and remain independent. St John has seen recent growth of services within the CHSP (Commonwealth Home Support Program), NDIS and Queensland Community Transport programs. We provide community transport, social support, telephone services, Medical Alarms and companionship services to both Queensland Seniors and NDIS community.

About the Role

This full time fixed term role is to provide support and coordination to the Client Engagement function of the organisation. The coordinator will record, coordinate and support the client, and provide administrative functions within the Community Services portfolio, provide operational support in meeting the overall objectives of building an engaged client base to ensure optimum delivery of all services and programs.

Duties and Responsibilities

  • Conduct needs assessments and advise consumers on St John Community Services, products, and programs
  • Manage incoming calls and maintain engagement with the clients as they access the services provided by the organisation
  • Manage and maintain client records, relevant data bases and other systems to effectively enable clients to register, use and exit services
  • Management of client information and feedback, including database management to ensure that data collection and related information provides comprehensive profiles, planning, case notes and any other important information
  • Identify and report to Management any gaps in Service Delivery that require attention, including workforce related needs
  • Liaise with relevant funders and government departments to assist intake of new clients into the organisation, and to aid in reassessment or referral to more appropriate services based on needs
  • Attend network meetings, forums, expos and workshop to develop interagency relations, and build knowledge in the community about St John Ambulance Qld
  • Assist in developing continuous quality improvement systems, and participate in providing quality services and support for customers
  • Assist with all other Community Service programs as required, training will be given to assist with the understanding of all Community Service programs
  • Encourage an environment that empowers service users and promotes positive behaviours, ongoing learning and independence

About You

  • Certificate 3 or higher in Aged Care, Disability or similar (preferred)
  • Demonstrated contemporary experience in client centred community based aged/disability/community care service delivery
  • Excellent telephone customer service skills
  • Experience working in a fast-paced call centre environment
  • Ability to acquire extensive knowledge of the St John products and services range
  • Demonstrated effective interpersonal and written communication skills; and ability to quickly establish rapport
  • Ability to maintain confidentiality on all issues in relation to St John, consumers and members
  • The ability to work independently and as a team member
  • Proven ability to manage schedules and tasks and meet personal, team and organisational KPIs
  • Hold or ability to obtain and hold an NDIS Disability Worker Screen
