Role overview

  • Provide non-clinical early intervention supports for vulnerable families with children and young people who are showing early signs of, or are at risk of developing mental illness
  • Ensure children and young people are at the centre of the service delivery model and their voices are heard and responded to
  • Provide short term assistance, information & referral
  • Provide community outreach & home visits
  • Provide longer-term (up to 6 months) early intervention for clients & families requiring more intensive support. This could include goal setting, mentoring and practical supports to help build strengths and resilience.
  • Development and implementation of Family Action Plans, inclusive of the child, young person & relevant family member/s, to identify and action consumer goals

Requirements

  • Minimum of Cert IV in Mental Health, Community Services or other related discipline (ideally a completed Diploma or Bachelors)
  • Knowledge, experience and skills in working with vulnerable children & young people within a family context, who are at risk of developing mental illness
  • Ability to work with marginalised individuals, families and communities
  • Ability to develop partnerships and linkages with other services to establish good referral pathways into and out of the service
  • Ability to work within a team, as well as autonomously and manage a case-load
  • Highly effective communication & interpersonal skills
  • Record Keeping, Case note & computer data entry skills
  • Must be eligible to apply/currently hold a Blue Card
  • Must hold/ be willing to apply for NDIS worker screening clearance 
  • Current Driver's License
