About the Service

The Matthew Talbot Hostel provides accommodation and specialised support to people who are homeless or at risk of homelessness.

The Role

The Team Leader - Case Management is an important leadership role that will provide guidance and support to clients and a team of caseworkers. The role's key responsibilities include:

  • leading the team in the effective delivery of case management services, with the ultimate goal of providing clients with sustainable housing.
  • being involved in continuous improvement activities e.g. accreditation, review of policies and procedures.
  • providing written reports on activities and case works on a regular basis.
  • ensuring that all required documentation is completed e.g. case management/client interactions.
  • conducting regular case plan reviews with clients.
  • guiding the team in developing programs aimed at developing clients' living skills and ability to engage with the community.

This role will report to the Area Manager, Homelessness and Housing Services - Inner City.

To be successful in this role, you will need:

  • Relevant tertiary qualification/s in Social Work/Social Science/Community Welfare or other relevant field or equivalent work experience.
  • Demonstrated experience in homelessness case management/outreach service delivery.
  • Experience in a supervisory role, with a good understanding of administrative, accountability, data management processes.
  • Highly developed interpersonal skills with an ability to relate positively and confidently with a wide range of people, must possess good negotiation and conflict resolution skills)
  • Knowledge of the housing & community services sector, child protection issues, and legislation.
  • Proficiency with computers and industry-related software is required.
  • NSW driver licence and flexibility to travel.
