Support Collective is an independent provider of Support Coordination providing Level 2 Support Coordination & Level 3 Specialised Support Coordination. We do not deliver other supports under the NDIS framework and are therefore able to offer a non-biased approach to service delivery. We work closely with our communities and collaboratively with mainstream services, to proudly deliver individualised services to our participants.

We are offering Part-time or Full-Time opportunity to the right applicant.

You will plan, develop, and coordinate services for your participants by:

  • Planning, developing and building capacity of individuals to increase their independence and manage their own supports
  • Liaising with providers and services to maximise participant outcomes
  • Monitoring and reviewing plans and goals in consultation with participants and their support networks
  • Providing support and assistance as appropriate to relevant others/stakeholders
  • Establishing and developing strong relationships with NDIS, community and mainstream supports
  • Keeping efficient and accurate records of your support for each participant

The successful applicant will have significant community services experience, with strong community connections and previous experience in a similar role.

Essential Qualities, Skills & Experience

  • Tertiary qualification and/or significant experience working in the Community Services sector (Disability, Mental Health, Housing, Justice)
  • A thorough understanding of the NDIS and relevant legislations, specifically in regards to support coordination and the importance of choice and control for participants
  • Strong interpersonal skills with an understanding of diversity when working with participants, their families, friends and support networks
  • Demonstrated ability to develop positive relationships with a range of participants, their families, friends and support networks
  • Well developed communication (both oral and written) skills with the ability to prepare reports and correspondence in clear and concise language
  • Computer literate and proficient in the use of the Microsoft Office applications
  • Ability to work autonomously, from home and as part of a strong supportive team
  • Drivers license and own vehicle
  • Valid NSW Working with Children Check
  • Valid NDIS Worker Check (or willing to apply)
  • Previous experience as a Coordinator of Support is highly regarded.
