VincentCare Victoria is seeking an appropriately-qualified person to take on the position of Key Worker who has experience and qualifications to work with homeless and marginalised single adults to provide a case management response.
The program aims to assist people in securing and maintaining affordable housing by addressing identified unmet needs via a case management framework. The Key Worker will work within a multi-faceted team based at Ozanam House.
The role is primarily focused on supporting clients in the Ozanam House crisis and short-term/extended stay accommodation. The Key Worker will be supporting clients through crisis, recovery and growth by means of supporting and referring clients with diverse support needs: mental health, substance misuse, social marginalisation, legal, family violence, intellectual and/or physical disability.
The Key Worker will aim to work from a collaborative, integrated and client centred approach with clients, and use ongoing assessment, case planning and review, and referral.

You are an appropriately qualified professional with experience in providing high level assessments, case planning and support response to people experiencing homelessness.
You have experience in supporting clients who are homeless, or at risk of homelessness, and who present with complex needs in a range of areas including: alcohol and other drugs, mental health, physical illness or disability, and psychosocial or behavioural issues.
