About the team

The Outreach Support Services Team currently has four separately funded programs and works with a range of client cohorts. The team supports single adults over the age of 25, couples and families that are experiencing homelessness or have complex issues.

The program aims to support and connect people experiencing homelessness with housing and support services to enable them to reconnect and re-establish. Outreach Support Services has two specialist portfolios; one that works with First Nations Elders and the other that supports people from the LGBTIQ community.

About the role

The Outreach Support Services Team has both SHS, HACC and CHSP funded programs that works with people 25 years and older who are currently homeless or at risk of homelessness. The team works from an outreach model to meet clients they are at. As Team Leader Outreach Support Services you will be overseeing the teams day to day work, finance, as well as data and risk reporting.

VincentCare Victoria is seeking an appropriately-qualified person to provide leadership, guidance, mentoring and supervision to a dedicated team of case managers who provide short, medium and long term case management to people experiencing homelessness.

Key to the success of this role is using a strengths-based and trauma-informed approach to. The role requires knowledge of NDIS, Child Protection, AOD and the Mental Health and/or Justice services.

About you

You are an appropriately qualified professional with experience in providing comprehensive case management, planning and support to assist clients within a homelessness setting.

You have an understanding of partnership initiatives and deliverables and an ability to work with and alongside other programs and organisations. You understand the importance of interacting in ways that respect the equality, diversity, choices, rights, safety and responsibilities of individuals throughout the process whilst also maintaining strong professional boundaries.

You have experience in maintaining constructive, empathetic, supportive and respectful working relationships particularly for an integrated service response.

Your experience and knowledge also extends to:

  • Effectively engaging with people experiencing complex issues
  • An ability to manage critical incidents, identify and act upon immediate risk of danger to self and others in both outreach and service based capacities.
  • Highly developed interpersonal, written and verbal communication skills
  • Experience in liaising with other community service organisations for the development of on-going relationships and referral protocols beneficial to client support

Salary Range: SCHCDSI Level 6 $46.91 - $48.99 per hour dependant on experience
