A Support Co-ordinator plays a critical role in supporting participants to navigate the NDIS system, understand their NDIS plans, connect into the broader system of services and to build their capacity to direct their supports independently over time.
As a Support Coordinator your role is to support NDIS clients by monitoring and coordination their plans. You will support clients to understand and implement their NDIS plan and link them to the community, mainstream and other services to enhance an individual’s quality of life.
This is a full time role based in our Liverpool hub.
You will have a relevant qualification and/or experience working within the disability sector and;
