We are growing and currently have a Support Coordinator vacancy in Sydney working with and supporting participants to achieve their NDIS plan goals. This is an opportunity to join the Co-operative Life supporting individuals living with Mental Health conditions to lead meaningful lives.

In your role as a Support Coordinator, you will be responsible for connecting people who have a National Disability Insurance Scheme (NDIS) Support Plan to access a range of supports. The role duties:

As a Support Coordinator, you will deliver high-quality transition support for people living with varying circumstances and levels of complexity. You will match participants with community-based and/or mainstream programs to fulfil their individual needs as per individual NDIS plans, whilst providing continuous support and assistance across the time-frame of each plan.

Professional Skills and Experience:

The successful candidate must have a good understanding of NDIS Support Coordination

  • Relevant tertiary qualification and/ or extensive experience in psycho-social support co-ordination
  • Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and the disability sector
  • Min 2 years of previous NDIS Support Coordination experience
  • Highly organised, strong time management skills, stress-resilient
  • Knowledge and ability to use Excel and have strong budgeting purposes
  • Well-developed communication and interpersonal skills including the ability to work and communicate effectively with participants, their families, allied health professionals, community agencies and other professionals
  • Ability to work in a very busy environment, work independently and as a part of a team
  • Valid Australian Driver’s Licence and car
  • Current NDIS Screening / Criminal History Check (cleared within the last 12 months)
  • Empathy and the ability to build strong relationships
  • Experience in mental health, physical and intellectual disabilities and also aged care home support.
  • Demonstrate experience in the documentation: Individual plans, risk management, incidents, daily case notes, staff reviews, performance management plans and reporting.

Interpersonal skills and experience:

  • Demonstrated capacity to collaborate as part of a diverse team and work autonomously.
  • High level of stress resilience and emotional intelligence.
  • Demonstrated ability to solve problems and use initiative.
  • Strong communication skills and demonstrated flexibility in methods of alternative and augmentative communication, particularly being able to use active listening skills.
  • Highly organised with excellent time management skills. Crisis management experience required

Conditions of Employment:

  • Terms and conditions of employment will be based on the Social, Community, Home Care, and Disability Services Industry Award 2010
  • Meeting clients and commuting will be part of the job routine
  • Some on-call work or after-hours work may be required in this position
  • This position will require work with vulnerable or at-risk clients. The support coordinator is expected to follow TCL’s WHS procedures and Mandatory Reporting obligations at all times in order to ensure the safety of staff and customers
  • A strong focus on work-life balance which means flexible working hours
