The Planning Collective represents a team of disability professionals who have come together to deliver Independent NDIS Support Coordination. Drawing on decades of experience and collective knowledge, we adopt an individualised, creative and collaborative approach to our work.

We operate as a network of independent contractors, working together under The Planning Collective's quality and business management framework. You set the pace and choose when and where you work, you also get to decide on the caseload you are comfortable with. There is no pressure to pick up more referrals, we believe in quality over quantity!

As an Independent Support Coordinator with The Planning Collective, you will receive:

About the role:

We are looking for two support coordinators to join our team across the Sunbury & Macedon Ranges or Mansfield & Murrindindi areas. Support Coordinators assist NDIS participants to realise the full potential of their NDIS plans. It’s a diverse and highly rewarding role, and we absolutely love our work. As an Independent Support Coordinator your role will include, but is not limited to:

  • Delivering NDIS support coordination and planning as defined in The Planning Collective’s support coordination policy
  • Working in partnership with the participant and their family where appropriate, to develop a support plan that identifies actions and outcomes
  • Coordinating services as defined in the participant’s NDIS plan, using a strengths- based and capacity building approach. This will include informal supports, mainstream services (housing, education, health), specialist services, and NDIS funded supports
  • Conducting risk assessments with each participant, and proactively managing identified risks
  • Empowering participant’s and their family by providing information about the range of mainstream and specialist services available to them, including advocacy services
  • Providing timely progress reports to the NDIA
  • Maintaining accurate case notes and records of billable hours
  • Establishing and maintaining effective links with the NDIA, community services and other support providers.

About you:

  • You hold qualifications in one of the following fields: education, psychology, allied health, disability, social work or healthcare
  • You have a strong working knowledge of the National Disability Insurance Scheme
  • You have a minimum of three years’ experience working in the disability sector
  • You have or are willing to get a current police, working with children check and NDIS Worker Screening Check
  • You have a valid driver’s licence and reliable car
  • You have an ABN and the relevant professional insurances
  • You are self-motivated, and strive for excellence
  • You are a kind and compassionate person, who genuinely cares about achieving positive outcomes for your customers.

Highly desirable:

  • Experience working in Local Area Coordination, complex case management, support coordination or planning with the National Disability Insurance Agency
  • You hold a bachelor's degree or higher in one of the following fields: education, psychology, allied health, disability, social work or healthcare.
