Homelessness Case Manager for SANS program, providing individual complex care for single people over the age of 18 with intensive complex needs.
The SANS Program is an outreach support program within The Salvation Army Homeless Case Management Services that works to provide support to individuals who are at risk of homelessness and have complex mental health issues. The programs objective is to support clients to enhance their wellbeing and independence. The program uses a strengths-based approach using The Outcome Star practice model with clients.
The SANS Case Manager will provide person centred supports ensuring access for clients to relevant services, support to obtain tenancies should they be housed, and advocacy in obtaining safe and secure housing if they are not, referral in relation to legal issues, income security, health issues, drug and alcohol problems, specialised counselling and psychiatric issues.
The Case Manager provides case management to individuals in an outreach setting. Case managers work collaboratively with clients and staff to ensure they are provided with appropriate support in their journey out of homelessness.
The SANS worker:
This is a permanent full time position, 38 hours per week.
Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.
