Seeking a proactive, person centered and experienced case worker to provide case management and outreach support to people who are experiencing AOD issues, with a focus on reducing the harm associated with their use.

You will successfully

  • Deliver evidenced based case management, counselling, education, advocacy and support to clients with complex needs
  • In collaboration with the client develop a holistic treatment plan that includes a range of therapeutic interventions that are person centered and family inclusive, to support the needs, health and wellbeing of the individual
  • Develop positive working relationships and referral pathways with external service providers to ensure effective coordination and continuity of care for the client
  • Deliver flexible, responsive and intensive outreach support
  • Maintain accurate and confidential client records, case notes and other data as required

This is a permanent part time position, 34.2 hours per week based at our AOD Service in Corio.

You will have

  • Minimum Diploma in AOD or related health and community services Diploma with the 4 AOD core units
  • Minimum 3 years experience within the AOD/Mental Health sector
  • Experience and resilience in working with complex AOD/Mental Health clients presenting with challenging behaviours
  • Current Victorian Driver's License
  • Current VIC Employee Working with Children's Check
  • Mandatory National Police check
