about the program

GreenLight Plus program housing support teams in Sacred Heart Mission and The Salvation Army will work with people making the transition from chronic homelessness and rough sleeping currently accommodated in hotel emergency accommodation due to COVID 19.

Complex Care Coordinators will work across the entire initiative with tier 3 (the most complex) clients supporting links to health and better chronic disease, AOD and mental health outcomes.

Access Health is a specialised AOD Primary Health Care Service that supports the health and other needs of people who use drugs. Access Health also aims to support people who may be experiencing homelessness, engaged in street-based sex work and/or experiencing difficulties managing their general health, mental health and chronic diseases.

Sacred Heart Mission, Salvation Army (TSA) Homelessness and TSA Access Health will deliver the exciting and innovative new multidisciplinary GreenLight Plus program. GreenLight Plus program teams in SHM and TSA will work with people making the transition from chronic homelessness and rough sleeping currently accommodated in hotel emergency accommodation due to Covid 19.

The GreenLight Plus Program will be delivered in alignment with SHM's exciting and innovative new Continuum of Care (CoC) project. The Vision for the CoC is to ensure that all clients receive the support and housing they need, to keep people out of the cycle of chronic homelessness.

about THE ROLE

We currently have an exciting opportunity within the GreenLight Plus Program for a Complex Care Coordinator to provide Complex Care Coordination including chronic disease, AOD and mental health care coordination to the stream 3 Greenlight Plus clients (the most complex). The role will work collaboratively with TSA and SHM Greenlight Plus teams to achieve positive client outcomes.

Responsibilities include:

  • Provision of practical, flexible, timely, respectful and non-judgmental services to tier 3 Greenlight Plus clients
  • Developing effective relationships with clients that encourage self-determination and participation
  • Working effectively with a specialised understanding of the impact of AOD and mental health
  • Development of care plans via active client participation
  • Provision of information, education, advice practical support and assistance to clients
  • Assisting clients to build on-going relationships with General Practitioners and other health professionals and services and supporting clients to receive thorough health assessment including identification of health goals
  • Advocating for clients needs within the health sector
  • Actively working to overcome the impact of stigma and discrimination within health services
  • Developing formal and informal relationships with local health and other community services

Reporting to the Program Manager this position is located across the southern, central and western metro Melbourne regions and integrated within the SHM and TSA Greenlight Plus housing support teams.

HOURS AND SALARY

This is a permanent part-time position, 30.4 hours per week.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

ABOUT YOU

  • Degree qualification in nursing, social work, or related field is essential
  • Experience working with people with complex needs including comorbid AOD and mental health is required
  • Practices with the understanding of the physical, emotional, psychological and behavioural impact of trauma
  • Understands the impact of AOD and mental health dual diagnosis on health and other outcomes
  • Practices with an understanding of the principles and benefits of a harm reduction approach when working with people who use drugs
  • Supports clients to set and achieve goals through establishing and monitoring a care coordination plan in consultation with clients and health professionals
  • A current and valid VIC Drivers License
  • Be able to provide proof of Eligibility to Work in Australia
