The Salvation Army Australia's Homelessness Stream provides high quality, person-centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.
The Housing Pathways Program (HPP) sits within the Western Metro Homelessness Service and provides housing services and support to connect with the housing system for those in custodial care in the metropolitan Melbourne and Barwon region of Victoria; specifically, the Melbourne Assessment Prison, the Metropolitan Remand Centre, Port Philip Prison, Ravenhall Correctional Centre, the Judy Lazarus Transition Centre, Thomas Embling Hospital, Barwon Prison and Marngoneet Correctional Centre (Karreenga).
We have an exciting opportunity for a values-driven and proactive Initial Assessment & Planning (IAP) Worker, reporting to the Team Leader, Housing Pathways Program at Barwon Region Prisons. The IAP Worker is instrumental in ensuring high-quality provision of housing, information and referral services to persons at risk of homelessness, within and exiting custodial settings in the Barwon region.
Service provision is located mainly at the usual workplace, the HPP office, situated at the Western Metro Homelessness Service in Sunshine. The Housing Pathways Worker will also attend Barwon Region Prisons - Barwon Prison, Marngoneet Correctional Centre (Karreenga). There may be times that service provision will be required at other custodial facilities managed by the Housing Pathways Program.
This is a full-time fixed-term position to 30 June 2023.
