GreenLight Supportive Housing program will work with people making the transition from chronic homelessness and rough sleeping to placement in longer-term housing to address their unique experience of homelessness and the individual health and wellbeing factors to maintain their housing. The key outcome is stable housing for people and no return to homelessness.

The GreenLight Supportive Housing Program will provide tailored brief intervention, floating support, and case management and service coordination services through a multi-disciplinary team, including supportive housing workers, mental health clinicians, and peer support resettlement workers to 684 people across the inner Melbourne area in 2019- 2020 with a focus on assisting people to stay housed. GreenLight will operate as a program team across the inner Melbourne area, with sub-teams employed by VC, SA, and SHM.

GreenLight will also be evaluated to demonstrate the efficacy of the model and share learnings via independent research and evaluation and internal participatory action research. GreenLight has been funded for two years through the Victorian State Government's Victorian Rough Sleeper Action Plan.

We have an exciting opportunity for a suitably qualified Case Worker to join our team. This is a permanent full-time position.

You will successfully:

  • Provide assertive engagement, brief intervention, case management and service coordination with people who have experienced chronic homelessness and/or primary homelessness.
  • Work to ensure that the service provided is person-centred, effective in achieving client goals, program outcomes, and are of the highest quality in accordance with Salvation Army's values and legislative and funding requirements.

You will have:

  • A tertiary qualification in social work or a related discipline is considered desirable.
  • Experience working in outreach capacity using assertive engagement practice
  • A current and valid VIC Drivers Licence
  • Valid VIC Employee Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia

Specialist Competencies

  • Client assessment: Uses appropriate information, tools and techniques to gather and analyse information; Identifies and prioritises appropriate actions including referral
  • Case Management: Supports clients to set and achieve goals through establishing and monitoring a case management plan; Identifies and implements case management processes appropriate to the needs of the client
  • Homelessness: Demonstrates knowledge of homelessness and housing pathways including context, causes, consequences and associated issues; Understands and applies homelessness models, approaches, theories and practices when dealing with clients
  • Behaviour Support: Identifies and uses appropriate strategies to deal with challenging or high risk situations including knowing when to deescalate and when to seek assistance
