    The Salvation Army Australia's Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

    THE ROLE

    As the Together Home Case Manager you will provide intensive outreach case management for participants who have a history of sleeping rough as per the Case Management Framework. Working with participants to support them in sustaining and maintaining their tenancy in Housing First Approach.

    Key responsibilities include:

    • Risk assessments and safety strategies are to be put in place prior to outreach support and home visits.
    • Trauma informed framework and housing first principles are applied
    • Adhere to 'Together home' guidelines
    • Case plans are developed alongside the participant and tailored according to each Participant's goals, background and needs
    • Outreach meetings are carried out in relevant timeframes
    • Care plans are implemented and reviewed in a timely manner
    • Wrap around supports are implemented and lead by the client, monitored and reviewed
    • Attend internal and external stakeholder meetings.

    Reporting to the Team Leader Case Management this position is located at Foster House, Surry Hills.

    HOURS AND SALARY

    This is a full time position, 38 hours per week.

    Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, Crisis Accommodation level 2.

    ABOUT YOU

    • Tertiary or other qualifications within community services , social work or related discipline (Minimum Diploma Level, working toward a degree)
    • Understanding of Trauma Informed Practice; Homelessness; AOD
    • Solid level of experience (minimum 2+ years) in the implementation of case management and systems for disadvantaged and/or complex needs clients.
    • Experience working with people from culturally and linguistically diverse backgrounds and religions.
    • Experience working in collaboration with other organisations to develop and tailor programs to meet clients' needs.
    • Working with Children Check
    • Current NSW Drivers license
    • Availability to participate in occasional weekend work
