About the role

  • 3 full time fixed term contract positions up until June 2022
  • This is a mobile role with flexibility to WFH and the successful candidate can be based across various locations on the NSW North Coast

The C2bMe@Home Mental Health Clinician role encompasses independent provision of comprehensive, evidence-based clinical care coordination, psychological and psychosocial interventions consistent with the model of care.

The Mental Health Clinician will be responsible for individual and group-based therapy, liaising with GPs, home care providers, LHD, local community organisations, and families and carers. They will support consumers in their psychological recovery from isolation aiming to optimise functioning to continue living with dignity.

Duties and Responsibilities include but are not limited to

  • Work as a part of the multidisciplinary team to ensure participants are identified, assessed and provided access to the service in a timely manner.
  • Deliver comprehensive and evidence based therapeutic interventions including in-home functional recovery activities in line with the Service Delivery Principles of C2bMe@home
  • Plan, manage and implement care plans and wellness plans in collaboration with the consumer, carer, and other members of the multidisciplinary team including GPs
  • Commitment to continued development of clinical skills, through participation in clinical supervision and use of evidence-based best practice according to the philosophy, standards, guidelines, and protocols of the service. 

The “Continuing to be Me” C2bMe@Home program is a comprehensive in reach person centred model of care designed to reduce the impact of physical distancing restrictions associated with Covid-19 on the mental health and well-being of older people living in the North Coast of NSW region

The C2bMe@home model uses trauma informed practice to reinforce identity and purpose, increases connectedness, develops hope, reduces symptomatology, and optimises functioning so that Older People can continue to live with dignity in the community. It offers low and medium intensity interventions through therapeutic groups & individual clinical care led by skilled health professionals

About You

  • Demonstrated assessment (excluding clinical placements) within the mental health field. Ideally with experience working with consumers experiencing mental health concerns.
  • Experience working within a multidisciplinary team environment and person-centred models of care including engaging positively with consumers and their families/carers, as well as other stakeholders.
  • Experience delivering contemporary, evidence-based practices and therapeutic interventions for older persons., while meeting data collection and patient record management requirements.
  • Ability to use relevant information technology, electronic medical record systems and data management tools
  • Knowledge of relevant legislation such as the Mental Health Act (2007)
  • Mandatory Influenza vaccine compliance
  • Experience in collaborative partnerships with community-based organisations
  • Experience working with specific populations of elderly people (e.g. Aboriginal Torres Strait Islander populations, CALD)
  • Previous experience in delivering therapeutic interventions for elderly people 65+ years or 50+ for Aboriginal and/or Torres Strait Island communities (desirable)

Essential Requirements

  • Occupational Therapist: The possession of a tertiary degree in Occupational Therapy
  • Registered Nurse
  • Psychologist: The possession of a tertiary degree in Psychology (or equivalent)
  • Social Worker: University level Social Work Degree, eligible for membership to the Australian Association of Social Workers (AASW)
  • AHPRA registration (if applicable)
  • Working with children check
  • Drivers Licence
