Family and Carer Mental Health Program of Uniting offers education, training and individual support services to families and carers of those with mental illness, aiming to improve care well-being, resilience, coping skills and relationships.

Role Overview:

The Individual Support Worker will work with the Team Leader to ensure that the most appropriate and effective services are provided to carers accessing the service. Support workers will support the creation of individualised support plans to best support carers in their Carer role.

Duties and Responsibilities include but are not limited to:

  • Assist with the assessment of Carers and the development of individual support plans based on individual Carer goals.
  • Assist with the monitoring and review of Carer support plans, in partnership with family members/clinicians/ community organisations as appropriate.
  • Liaise with specialists and other to ensure consumers receive timely and relevant support, information and referrals
  • Ensure Risk management practices are considered and followed in respect of clinical governance and WHS Management

Essential Criteria:

  • Experience of providing individual advocacy for families and carers of people with a mental health issue
  • Understanding and application of the recovery model
  • Demonstrated ability to liaise with other organisations/health care professionals who are involved in consumer care
  • Practical knowledge and understanding of issues facing people with a mental illness and their carers
  • Demonstrated ability to work cohesively within a team, as well as independently to achieve KPIs
  • A current full driver’s license
