ABOUT THE ROLE

The Integrated Family Support Service's Case Manager works in collaboration with other programs to provide services that wrap-around participants and enable them to make, implement and maintain positive decisions about their lives.

The Case Manager works under general direction from the Team Leader - Family Support Services to provide short-, medium- or long-term support to individuals and families who are dealing with issues that affect the quality of their lives.

To work effectively, the Case Manager engages in assessment, goal setting, capacity building, networking and referral using methods that empower people and build their resilience.

ABOUT YOU

We are looking for candidates with the following qualities:

  • Maturity – resilient and positive outlook, with a clear set of values that include justice, honesty, and integrity.
  • Communication - high level communication, negotiation, networking, and relationship- building skills.
  • Collaborative practice – work collaboratively and have a clear understanding of the ethical boundaries in case management.
  • Strengths-based approach – assess and support participants to journey towards reaching their goals.

Essential Criteria

  • Relevant qualifications in social work, psychology, counselling or similar and minimum one year experience in human services delivery.
  • Understanding of and a commitment to work within the ethos of the Uniting Church in Australia.
  • Highly developed case management skills including the capacity to work with children, young people and adults.
  • Demonstrated capacity to identify, connect to and maintain relationships with participants, stakeholders, staff, volunteers, and students.
  • Highly developed capacity to work both in a team and independently.
  • Evidence of strong interagency networking skills and community development practices
  • Demonstrated skills in time management, setting priorities, planning, organising, reporting and achieving outcomes.
  • Working with Vulnerable Persons card, police check and driver’s licence, or capacity and willingness to get them at your own expense.

Desirable criteria

  • Certification in Tuning into Kids, Tuning into Teens, Circle of Security, Bringing Up Great Kids, Mental Health First Aid, Cultural Awareness training, Domestic Violence training, Trauma Informed training, and other role relevant certifications.
  • Capacity to use a person centred, and strengths-based approach.
  • Familiarity with the use of technology including appropriate computer software to fulfil the responsibilities of the role.
  • Good knowledge of the ACT community sector.
  • Clear understanding of Workplace Health and Safety guidelines and how to apply them in practice.
