The Out of Home Care Case Manager will provide Key Work/ Case Management for Clients of Vacca’s Better Futures and Targeted Care Packages programs. The position is intended to be able to follow Clients across their care journeys from TCP’s to the Better Futures program. Building lasting relationships through empathetic and trauma informed service deliver.
