About the role

The Out of Home Care Case Manager will provide Key Work/ Case Management for Clients of Vacca’s Better Futures and Targeted Care Packages programs. The position is intended to be able to follow Clients across their care journeys from TCP’s to the Better Futures program. Building lasting relationships through empathetic and trauma informed service deliver.

About You

  • Certificate, Diploma or Degree qualification in Community Services, Social Worker and/or substantial experience in these fields is desirable
  • A demonstrated knowledge and understanding of issues surrounding Youth, homelessness, adolescent out-of-home care, the Child Protection and Youth Justice systems and available resources and services Eastern Region.
  • Skills in assertive outreach, assessment and case management.
  • Demonstrated ability to engage young people in out-of-home care including Residential Care, Kinship Care and Home-Based Care.
  • Demonstrated ability to assist young people transition and develop the necessary skills for independent living.
  • You must have and continue to hold a full Victorian Driver’s Licence, a current employment Working with Children Check card and a National Police Check.
  • Current COVID-19 vaccinations
