Intake & Assessment Coordinator

Very Special Kids
  • Casual role - Registered Nurse, Grade 4A
  • M-F Office hours

Very Special Kids provides holistic care for Victorian families facing the potential death of a child with a life-limiting condition. We provide counselling and emotional support for the whole family, medically-supported respite care and end of life care and long-term bereavement support. Our free-of-charge services help more than 850 families across Victoria, with ongoing support, from the child’s diagnosis all the way through to bereavement or the child joining adult services.

Our eight-bed children's hospice offers planned respite and emergency respite as well as end-of-life care for children with life-threatening conditions. Most children who come to stay have complex medical and care needs.

In 2021 we are embarking on our single biggest service improvement project; a hospice rebuild to deliver a new world-class centre and ensure the highest level of internationally led best-practice care can be delivered. During the time of the build we will be temporarily based in Monash Children’s Hospital.

To complement our team and support during periods of leave, we have a rewarding casual role in our Hospice Management team. As the Intake & Assessment Coordinator, you will develop strong connections with external referrers and process all external referrals. In partnership with the Family Support team, you will complete assessments with newly registered families and coordinate the Hospice Booking system for all families accessing the Hospice.

To be successful you will be a Registered Nurse with advanced assessment, intervention and review skills and a commitment to the provision of family-centred based care. Your communication and interpersonal skills will be well-honed coupled with advanced computer skills including database management and administration.

Very Special Kids offers its employees a supportive and rewarding environment including a supportive team, unique beautiful setting conveniently located to shops, parking and transport in Malvern, salary packaging, health & wellbeing initiatives and targeted individual development plans. Note, the role will be based at Monash Children’s Hospital during the rebuild of the Hospice due to commence in August 2021 for the period of the rebuild project.

At Very Special Kids, our goal is for our workforce to reflect the diverse community we serve. We continually seek to employ people of any gender, age, religion, sexual orientation, with a disability, Aboriginal Australians and varied cultural backgrounds. If this sounds like you, please take a look at the position description below or on our website www.vsk.org.au.

Very Special Kids is committed to providing a child-safe environment and requires all staff to have Working with Children and Police Record Checks.
