The role

Reporting directly to the Primary Care Navigation Manager, this role is responsible for providing state-wide virtual service to practices that builds capacity, provides direction with quality improvement to achieve Commonwealth Performance Quality Framework (PQF) and contributes to the centre of excellence for information and resource provision.

This role will be offered on a part-time, (30 hours per week) limited term contract basis to 29 June 2022 and provides the flexibility to work from both home and office as required.

Responsibilities will include;

  • Proactive contact with General Practice regarding quality improvement (QI) information and coaching around data reports and identifiable changes required in practices.
  • Maintain and contribute to the resources and information required to build capacity in general practices and primary care.
  • Respond to or coordinate requests for support and training.
  • Promote & assist practices to improve quality care including working to achieve Performance and Quality Framework (PQF) targets such as – immunisation rates, cancer screening rates, promoting & improving the uptake of practice accreditation, assisting practices to use eHealth systems etc.

About you

To be considered for this role, candidates must have demonstrated experience in a similar role and meet the following criteria:

  1. Recent, extensive General Practice experience and knowledge of primary care operations.
  2. Excellent interpersonal communication skills, including a focus on positive customer service both verbally and in writing, and the ability to deal with stakeholders from a range of cultural/linguistic backgrounds and healthcare roles.
  3. Proficient at using virtual processes to engage and influence quality improvement in general practice.
  4. Experience using relevant software such as Customer Relationship Management (CRM) and/or ability to learn new systems quickly.
  5. Demonstrated ability to analyse and interpret reports particularly general practice data reports.
  6. Highly developed organisational and planning skills.
  7. Ability to work independently, show initiative, problem-solve and work productively as part of a team.
  8. Ability to adapt to a changing environment and undergo upskilling as required.
  9. Knowledge of rural and remote primary care in Western Australia.
