About this Position

The Team Leader will oversee and coordinate the day-to-day operations of the Care Coordination team in Kings Cross. The Care Coordination team provides proactive and comprehensive support to visitors with multiple intersecting and complex needs including matters related to housing, substance addiction, physical and mental health, domestic violence, and legal issues.

You will have experience in leading a team to deliver broad ranging support services as well as experience designing, implementing, and refining frameworks of care.

The Team Leader reports to the Operations Manager - Programs and is a member of the Programs operational leadership team. This team is going through an exciting period of growth and change including a recently implemented new model of care.

You will be adept at managing multiple priorities and timelines and be able to move seamlessly between operational planning and team management and support. You will understand the issues faced by some of the most vulnerable people in the community and have a deep passion for this work.

Key Responsibilities

People Management, Leadership and Culture

  • Provide direction and regular supervision to team members and students as required.
  • Make decisions about day-to-day operations regarding staffing and events on the day and updating Operations Manager on important issues.
  • Facilitate regular case reviews with the team, ensure that staff monitor and respond to visitor referrals from other Programs and collaborate with relevant service providers as appropriate.
  • Lead the team in working effectively in the joint management of visitors’ care. Promote knowledge sharing and opportunities for cooperation within the Community Services team and with staff from other Programs teams.
  • Lead and foster a culture in line with Wayside’s values and professional industry standards including visitor focused and outcomes-centred care.
  • Provide opportunities for team members to debrief about visitors and facilitate referral to professional support as required.
  • Be the spokesperson for the team and respond to emails and inquiries from other teams, service agencies and members of the Wayside community.
  • Develop team members through skills based training and effective work planning.
  • Lead the team in working effectively in the joint management of visitors’ care. Promote knowledge sharing and opportunities for cooperation within the Community Services team and with staff from other Programs teams.
  • Lead and foster a culture in line with Wayside’s values and professional industry standards including visitor focused and outcomes-centred care.

Service Design and Implementation

  • Contribute to the development of Kings Cross Care Coordination through strategic and operational planning, and delivery of those plans. This includes reviewing policies and procedures and assisting with the design and implementation of new service systems and models of visitor care.
  • Promote collaboration, establish effective partnerships and working arrangements with other programs within Wayside for the advancement of visitor care objectives. This includes ensuring effective referral pathways among the services and programs.
  • Actively contribute to the objectives of the Wayside’s Visitor Services Plan through participation in Senior Operations meetings and Programs working groups.
  • Participate in, or delegate participation in, interagency meetings and ensure Wayside offers a consistent and professional presence. Explore opportunities to expand relationships and collaborative work with local service providers.

Administration, Reporting and WHS

  • Provide statistics and reports on the operations of the Care Coordination team for Board reporting, grant acquittals and other purposes as required.
  • Ensure that staff are recording accurate and timely data and information on operations. Develop and implement effective processes and make use of available systems including CIMS.
  • Manage staff rosters, approve timesheets and manage expenses for Care Coordination operations.
  • Accountable for complying with legislation and all Wayside Work Health and Safety policies/procedures and instructions, reporting any hazards or safety issues to your manager and performing all tasks in a manner which guards against risk to self and others within the workplace. Undertake risk assessments

About you

  • Tertiary qualifications in a related discipline and/or relevant training and experience in managing Community Services programs.
  • Extensive experience working with at-risk populations and understanding of related issues, both practical and systematic (including but not limited to Juvenile Justice, Aboriginal and Torres Strait Islander, CALD, trauma, homelessness, mental health).
  • Experience in service design, planning and implementation.
  • High level interpersonal communication and problem-solving skills and the ability to resolve conflict within team and between visitors.
  • Ability to effectively communicate and collaborate with government and other service providers for the benefit of visitors’ health and wellbeing.
