Are you looking for a rewarding career that aims to help others access support and achieve their goals? As a Support Coordinator, you will work directly with NDIS participants to develop plans to support them to live a fulfilling and meaningful life in the community and achieve the goals set out in their NDIS plan. This involves coordinating a variety of services for participants, facilitating effective working relationships and communications with the carers, family, external services and other key stakeholders to help provide the most well-rounded support available for participants.
The Support Coordinator will ensure that Support Coordination services are delivered at consistently high standards of quality that meet and exceed both internal and external benchmarks and standards. In line with industry standards, it is expected that Support Coordinators attribute at least 80% of their time to providing funded supports.
The Support Coordinator forms part of the broader regional CDS team and works in close collaboration with this and other CDS teams across Wellways, as well as a wide range of other internal and external stakeholders, including the CDS Rostering Team, CDS Claims Staff, Quality and Service Development Team, regional colleagues, Local Area Coordinators, mainstream services and more.
The option exists for this opportunity to be converted to a part time (4 days per week) position for the right candidate. Pro rata salary will apply.
