As a Support Coordinator, you will work directly with NDIS participants to develop plans to support them to live a fulfilling and meaningful life in the community and achieve the goals set out in their NDIS plan. This involves coordinating a variety of services for participants, facilitating effective working relationships and communications with the carers, family, external services and other key stakeholders to help provide the most well-rounded support available for participants.

The Support Coordinator will ensure that Support Coordination services are delivered at consistently high standards of quality that meet and exceed both internal and external benchmarks and standards. In line with industry standards, it is expected that Support Coordinators attribute at least 80% of their time to providing funded supports.

The Support Coordinator forms part of the broader regional CDS team and works in close collaboration with this and other CDS teams across Wellways, as well as a wide range of other internal and external stakeholders, including the CDS Rostering Team, CDS Claims Staff, Quality and Service Development Team, regional colleagues, Local Area Coordinators, mainstream services and more.

Key responsibilities include, but are not limited to:

  • Working with a designated caseload to implement participant NDIS plans
  • Ensuring that all operational and administrative requirements are completed including regular reporting and records maintenance
  • Engaging participants in developing professional and trusting relationships
  • Promoting the participants' right to choice and control by enabling them to have direct input in how their services are delivered within their identified plan
  • Engaging stakeholders to undertake services as per participants' requests
  • Maintaining productive working relationships with stakeholders to ensure participant needs are met throughout the life of the plan

The option exists for this opportunity to be converted to a part time (4 days per week) position for the right candidate. Pro rata salary will apply.

What do you need?

  • Minimum Certificate IV in Mental Health or Disability or Certificate 4 in Aboriginal and/or Torres Strait Islander Health Care (Practice)
  • Excellent interpersonal and communication skills
  • Current valid Driver's License
  • The right to work in Australia, Working with Children Check, National Police Records Check and NDIS Worker Screening check
  • NDIS Worker Orientation Module Certificate
  • Evidence of full COVID vaccination or valid medical exemption
