Consumer Directed Services (CDS) refers to services which are led and funded by individual consumers. This includes services such as the National Disability Insurance Scheme (NDIS) services, Lifetime Care, and Community Care. The services provide support, care and access to services and technology for Australians with disabilities as well as their families and carers.

About the Role

We are seeking a Team Leader to join our supportive team at Wellways Australia. In this role as Team Leader, you will be responsible for overseeing the day-to-day operations of the CDS Program across the Sydney region. Reporting to the Program Coordinator, you will provide operational leadership to the team, always acting as a positive role model to ensure participants are supported to live a fulfilling and meaningful life in the community. You will also work with various external stakeholders including the NDIA, other disability service providers, families, and heath professionals within the circle of participants.

Key responsibilities of this role include, but are not limited to:

  • Ensuring high quality outcomes are achieved for the service in line with client care plans and organisational requirements
  • Planning and completing service agreements and schedules of supports with participants
  • Supporting NDIS participant plan reviews
  • Rostering CDS shifts
  • Recruiting, onboarding, supporting, training and supervising CDS staff
  • Leading practice support relating to complex work
  • Monitoring intake and capacity
  • Facilitating team meetings and practice discussions
  • Motivating, supporting, and mentoring team members
  • Monitor resources and make sure they are always sufficient
  • Ensure all client paperwork is completed to a high standard and signed off
  • Make client appointments and communicate appropriately
  • Working collaboratively with the Clinical Care Consultant, APO representative and wider leadership and CDS teams
  • Maintaining a high level of program quality and driver continuous improvement

What do you need?

  • Minimum of Certificate III in Community Services or related and/or relevant experience
  • Experience preferred but full training will be offered to the right candidate
  • Strong interpersonal and communication skills
  • Appropriate IT skills
  • Satisfactory Police Records Check (no older than 12 months)
  • NSW Working with Children Check
  • Right to Work within Australia
  • Current valid drivers license
  • NDIS Worker Orientation Module
  • NDIS Worker Screening Check
  • Full COVID-19 vaccination Booster Certificate or valid medical exemption
