The Way Back Support Service (TWBSS) program is a suicide after-care service that has been developed to care for people discharged from hospital after a suicide attempt or crisis. The TWBSS programs hope to prevent deaths by suicide by providing care coordination and supporting participants to link with supports that help keep them safe and support recovery.
The role of the Support Coordinator is to participate in intake and assessment processes and support those who have been referred to Wellways from Latrobe Regional Hospital and Beyond Blue.
You will provide non-clinical care and individualised practical support to people and their families who are referred to the service for the first three months following a suicide attempt.
As one of our valued support Coordinators you will be assisting Way Back participants to develop their safety and support plans, you will also provide support in the first three months following a suicide attempt to help an individual to stay safe and connect with essential services.
Engage with and deliver safe and effective service to vulnerable groups who are known to be at high risk of suicide as well as reduce the burden on supporting networks, including family and carers by supporting individuals to self-manage distress and increase social connectedness.
You may be required to travel across the Gippsland region.
